Counselor Activity Log

Here is a Google Form for digitally tracking every contact with an additional section for treatment planning for students and beneath is a Google Sheet I use to print out a hardcopy and write it down quickly, so that I can later type it into the form with the same information as the google form.

Here is a generic Google Form that logs all activities. I have not found this one as useful in my own practice, but can be beneficial and editable if someone else finds this information helpful.
